Adults Social Work Assistant

Belmont Recruitment are currently looking for an experienced Social Care Practitioner to join Barnsley Council’s Hospital Social Work Team on an initial 6 month temporary contract. This is a full-time role working 37 hours per week, Monday to Friday.

Key Duties:

You will be responsible for the completion of Care Act Assessments to facilitate a smooth transition for our service users when they leave hospital and for supporting discharges from intermediate care bed / inpatient rehab settings. Therefore, knowledge and experience in the completion of adult social care assessments is essential. As part of this role, you will have input in discharge planning with health colleagues, but also to ensure that the right pathways are achieved for the individual and a strengths-based approach is promoted as standard.

The role entails working closely with individuals, families, carers and community services such as reablement, telecare, care homes and domiciliary home care providers to help individuals promote independence and safety when they leave hospital / health settings.

Requirements:

* To join us, you’ll need a relevant Level 3 Qualification in Health and Social Care.

* You’ll bring some relevant experience of working with adults, have knowledge and experience in producing quality assessments and experience of collaborating with various agencies.

* The ideal candidate will need to have some experience and understanding of the current Discharge to Assess process in relation to assessing / supporting people when they leave hospital. It is essential that you have experience of working collaboratively with health colleagues, as the role does entail working in partnership with health in various different acute and intermediate care settings.
