Job Description
Job Requirements of the Licensed Clinical Social Worker:
• Perform Biopsychosocial Assessments
• Assesses patient and plan appropriately to assist with social determinants of health
• Suicide Risk, Mental Health, and Crisis Assessment and intervention
• Document safety plan and other plans of care and care given in patient’s medical record
• Provide information and completion of Advanced Directives
• Re-assesses patient condition and revises plans-of-care based on problems that the LCSW identifies
• Provide resources when a veteran has been determined to be homeless
• Able to initiate, prioritize, and reorganizes appropriate measures in emergency situations
