To assess and care manage the complex needs of older people and their carers to ensure their health, wellbeing and social inclusion.
To liaise with Reablement and other professionals to ensure seamless care pathways which safeguard and promote independence.
To promote partnership working which supports the person centered outcomes to be delivered for older people.
To undertake safeguarding investigations as required.What we are looking for in a Social Worker:
Social Work England registration
Eligible to work in the UK
Hold a full UK licence
Post-qualified experience
