Key Responsibilities:
Work closely with young adults leaving care, assessing their needs and aspirations.
Develop and implement comprehensive pathway plans, promoting independence and well-being.
Provide practical support, advice, and advocacy to care leavers on education, employment, housing, and health matters.
Collaborate with external agencies to create a network of support for care leavers.
Maintain accurate and up-to-date records, ensuring compliance with statutory requirements.About You:
Qualified Social Worker registered with Social Work England.
Experience in working with care leavers, understanding the challenges they face.
Strong knowledge of relevant legislation and policies pertaining to leaving care services.
Excellent communication skills and the ability to build positive relationships.
