Position Overview: We are seeking a dedicated Transition Teacher to join our team. The Transition Teacher will play a pivotal role in facilitating a smooth transition for students moving between different phases of their educational journey, ensuring continuity of learning and personalized support.
Key Responsibilities:
Develop and implement transition plans for students entering or exiting our institution, collaborating with teachers, parents, and support staff to address individual needs.
Provide guidance and support to students in navigating academic, social, and emotional challenges during periods of transition.
Foster a positive and inclusive classroom environment conducive to successful adaptation and integration.
Monitor students' progress and provide timely feedback to facilitate their transition journey.
Collaborate with colleagues to align curriculum and teaching strategies across different educational levels, promoting consistency and coherence.
Stay informed about best practices in transition education and contribute to the continuous improvement of transition programs.
Qualifications:
Bachelor's degree in Education or related field (Master's degree preferred).
Valid teaching certification/license.
Proven experience in transition education or related field.
Strong understanding of child development and educational psychology.
Excellent communication and interpersonal skills.
Ability to collaborate effectively with diverse stakeholders.
