Are you a dedicated and compassionate Clinical Pharmacist seeking a fulfilling role within a forward-thinking, innovative, and progressive Practice?

We have partnered with a well-established training practice in rural East Sussex, who work in partnership with a social enterprise organisation who are a leading provider of urgent care in the South-East. Based at a unique, purpose built, eco-surgery, they have a list size of approx. 5,000 patients and are committed to delivering high quality, integrated primary care to support their patients to live their lives to the full.

Position Details:

* Job Title: Clinical Pharmacist

* Job Type: Full time/part-time hours available from Monday – Friday

* Location: Wealden District of East Sussex

* Team: Supported by GP Partners, salaried GPs, Care Co-ordinator, Social Prescriber, Mental Health Practitioners, Pharmacists and ANPs plus clinical administration team.

About You:

* Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.

* Experience as a pharmacist, demonstrated within a practice portfolio.

* An awareness of common acute and long-term conditions.

* May hold or be working towards an independent prescribing qualification.

* Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate.

* Excellent interpersonal and communication skills, and be organised, patient and empathetic.

Main Duties:

* Conduct long-term condition clinics and clinical medication reviews.

* Make recommendations for medicine optimisation and safety.

* Support staff with prescription and medication queries as well as manage repeat prescriptions and monitoring tests.

* Lead medicines optimisation and quality improvement.

* Provide training on therapeutics and medicines optimisation and critically evaluate literature and ensure compliance with CQC standards.
