Job Purpose and Role:

To deliver a Leaving Care Service to young people leaving care in Wolverhampton in accordance with the requirements of:

* the Children (Leaving Care) Act 2000,

* the Children Act 1989, the Children Act 2004, and other relevant legislation;

To provide support to young people using the service; to address young people's specific individual needs as identified in their Pathway Plans, working within a casework framework, to hold a case load; to work directly with young people individually and through group-work and activities.

Principal Duties and Responsibilities:

* Hold responsibility for a caseload of young people who are Care Leavers (relevant and former relevant young people).

* To co-work Eligible Children in care with Social Workers from the Children In care Service.

* Complete Pathway Plans and Reviews for young people as required.

* Address the specific individual needs of young people as identified in their Pathway Plans.

* Continuously evaluate and review the service provided for Care Leavers to ensure that standards of care are maintained and that they experience good outcomes.

* Complete allocated tasks within time scales. Ensure all statutory requirements are met in respect of allocated Care Leavers. Ensure that all casework decisions are implemented promptly.

* Attended statutory Pathway Plan Reviews, Planning Meetings, working parties, training courses, and other meetings as required.

* Work directly with young people to ascertain their wishes and feelings for all plans relating to them and to encourage them to be involved in the planning process as appropriate.

* Gather information and prepare documentation for Pathway Reviews for young people as required.

* Contribute to the delivery of group sessions, activities and personal development programs for young people using the service. Participate in these young people's groups together with colleagues.

* To provide Duty cover as required by the Team Manager.

* Support Foster Carers and residential staff to prepare young people for leaving care and to continue to provide young people with practical, emotional and social support as they make the transition from care to independent adulthood.

* To liaise with Children Service colleagues, other council service areas and external agencies.

* To actively engage in joint work with Connexions, the Youth Service and other relevant organisations and agencies.

* To support and monitor Care Leavers in further and higher education to promote educational achievements.

* To support clients within family and social contexts as appropriate.

* To assist in the development of packages of support in respect of Looked After Children receiving Aftercare support placed Out of City.

* To maintain casework records and provide reports as required by Children's Services procedures and as requested by the Team Manager or Senior Management.

* To engage in home visiting, casework and attendance at relevant meetings as appropriate to the Young Persons Advisor duties.

* To attend meetings and engage in personal development as directed and through the Employee Performance Review Scheme.

* To liaise with other services and agencies and promote work in partnership with colleagues when appropriate.

* To act in accordance with the Council's Constitution and other Codes of Conduct.

* To participate in staff development, appraisal and training as appropriate, including continuous professional development.

* To comply with the Council's agreed policies and procedures including but not limited to Health and Safety, and Equal Opportunities Policies, the Data Protection Act, Freedom of Information Act, Financial Management Regulations and other relevant Council and Government Regulations, Directives and City wide priorities.

* To undertake any other tasks, duties and responsibilities as directed and appropriate to the grade and role of the post subject to any reasonable adjustments under the Disability Discrimination Act 1995 as incorporated into the Equality Act 2010.

* To participate in the wider development of the service and contribute to service improvement as required
