Seeking a Registered Nurse to become our Home Health Case Manager in the Southern Kansas City, MO areas including Raytown, Belton, Grandview, Raymore, Peculiar, Lee's Summit, Pleasant Hill, Northland and Lee's Summit areas.
As a home health care case manager, your job is to provide hands-on patient care within a patient’s home, coordinate the services of other physicians or health care providers, communicate with health insurance providers, and supervise other nurses who visit the home.
Pay Range: $65,000 - $90,000 
Shift Availability: Full Time, Monday - Friday: 8am - 5pm. This position carries On Call responsibilities every 8 -12 weeks.

Responsibilities
Management of caseload including supervision of LPN visits
Varied weekly caseload includes routine visits, evaluations, and starts of care, allowing for day-to-day patient interaction.
Knowledge to provide education on health conditions including CHF, COPD, Diabetes, Wound Care, and Medication Management
Communication with physicians to obtain orders and write diagnosis driven plans of care.
Work alongside strong therapy team including PT/OT/Speech.
Requirements
Missouri or Compact Registered Nursing and Driver's License.
Terms must be met to receive Sign on Bonus.
Strong organizational and self-management skills
Strong and compassionate customer service skills.
Valid driver's license.
