Responsibilities & Duties:
Evaluate and treat patients using the most current technology and practices
Communicate with the physician to decide the initial plan of care based on initial patient assessment & physician’s orders
Successful management of a consistent case load
Participate in clinical outcomes monitoring, follow up, and agency performance improvement initiatives
Manage and educate Home Health Aides in implementing nursing services
Ability to work in a collaborative environment with an interdisciplinary team
Ability to travel San Antonio and surrounding area
Requirements
Current and unencumbered license to practice as an LPN in the state of TX
Minimum of one (1) year of nursing experience
Current CPR certification
Reliable transportation, valid drivers license with no DUI's, revocations, or suspensions in the last 5 years
exceptional customer service
Excellent written and verbal communication skills
MS office skills (Word, Excel, PowerPoint)
