Job Title: Senior Support Worker - Supported Living

Location: Greater Manchester

Role Overview: As a Senior Support Worker, you will play a vital role in our Supported Living team, providing holistic support to individuals with learning disabilities and mental health needs. Working closely with the Service Manager, you will ensure the delivery of high-quality care that enhances the overall well-being and independence of our residents.

Key Responsibilities:

* Complete health care tasks to a high standard

* Lead by example, providing guidance and mentoring to the team to deliver person-centred care that meets individual needs and preferences.

* Develop and implement care plans in collaboration with residents, their families, and multidisciplinary teams, ensuring goals are realistic, achievable, and regularly reviewed.

* Foster a positive and inclusive environment that promotes independence, choice, and social inclusion.

* Monitor and record residents' progress, behaviours, and any changes in health or well-being, reporting concerns promptly to the Service Manager.

* Participate in staff meetings, training sessions, and supervision to enhance professional development and ensure compliance with organizational policies and procedures.

Qualifications and Experience:

* Previous experience working in a similar role within the social care sector, ideally with individuals with learning disabilities and mental health challenges.

* NVQ Level 2 or 3 in Health and Social Care (or equivalent) is desirable.

* Good demonstration of English reading and writing skills

* Flexible approach to work

* A commitment to promoting dignity, respect, and equality for all individuals.

* Be able to effectively communicate
