Senior Social Care Practitioner - Transition Team

About the Role: OFFER SPONSORHIP

In this position, you will collaborate with individuals, their caregivers, professionals from other agencies, service providers, and Support Brokers to assist vulnerable adults and their families in identifying and coordinating services that cater to their needs while promoting independence, choice, and control. Managing a caseload of individuals with more challenging and complex needs will be a routine part of your responsibilities.

You'll need to be confident in conducting safeguarding inquiries, performing assessments in the community, and collaborating with diverse groups of individuals and partner agencies.

You'll receive professional supervision and work alongside a team of motivated, dedicated, and supportive colleagues.

Shortlisting Criteria:

To be considered for this role, your application must demonstrate the following:

* A recognized Nursing or Allied Health Care qualification

* Experience in Adult Social Care, including working with various groups such as older adults, those with physical disabilities, and those with learning disabilities

* Knowledge and application of current legislation, including the Care Act and the Mental Capacity Act, in a social care context

* Excellent communication skills, with the ability to communicate clearly, fluently, and concisely in a jargon-free manner, while maintaining a courteous, calm, and professional demeanor

* Willingness and ability to travel throughout the county to fulfill the demands of the role
