Permanent, full time (37.5hpw), rota working day shifts only.

As our Support Coordinator, you’ll be at the heart of supporting our customers who have complex mental health & homelessness needs to develop their skills to live more independently in the community. You’ll make a big difference each day as you help our customers achieve their hopes and aspirations. Amazing we know!

Typical day as a Support Coordinator.

Creating support plans with your customers and coordinating our team of Support Workers working to the plans, attending MDT meetings (Multi-Disciplinary Team), enriching relationships with external partners, such as; safeguarding, police, probation and housing.
Customer group – At the Tyneside Foyer you will support young people who are 16-25yrs and experiencing homelessness, at Hawthorn House the customers are male and over 18yrs.
Helping customers with daily living such as; making meals, attending appointments eg: probation or job centre, maximising income, maintaining their tenancy and moving onto independent accommodation.
Carrying out risk assessments, support planning, goal setting and regular reviews.
There’s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring

Passion to support our customers to live their best life, working collaboratively with an eye for detail.
Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
Experience of coordinating and assessing customer referrals.
The ability to work on your own initiative, remain calm under pressure and have a resilient approach. For the Tyneside Foyer you will need to have experience supporting 16-18yrs due to our services OFSTED regulation.
Due to limits on our current certificates of sponsorship, we are not able to offer sponsorship to external candidates for this role.

Our Team

You’ll join our outgoing team, full of diverse characters, who are all friendly and helpful with newbies. The Tyneside Foyer team is led by our Senior Client Service Manager Emma, who has worked for Home Group since 2017 when she started as a Support Worker and took full advantage of all the training and experience that the company has to offer. She is supported (and couldn’t do without) our 2 Client Service Managers, Scott and Jodie who, have all have a wealth of service knowledge to support the teams. 

Job Details

At both Tyneside Foyer and Hawthorn House, we provide 24-hour support to our customers. You’ll work on a 4-week rota basis that is completed for the full year, working 8am – 8pm, including weekend work and Bank Holidays. 
We do get together regularly for training and team meetings.
We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you!
Confidently able to use technology for creating and updating support plans, report incidents & to record KPI’s, complete online learning and to communicate with colleagues. 
You’ll need an Enhanced DBS check done and we pay for that.
