Social Worker - Hospital Discharge Team

Salary: £35 per hour

Doncaster Health Trust is seeking a dedicated Social Worker to join our Hospital Discharge Team. This role offers a unique opportunity to play a crucial part in ensuring seamless transitions for patients leaving hospital, promoting their well-being and independence.

Key Responsibilities:

Conducting assessments and developing discharge plans for patients.
Liaising with healthcare professionals, families, and external agencies to coordinate safe and timely discharge.
Providing support and advocacy to patients and their families during the discharge process.
Collaborating with multidisciplinary teams to ensure continuity of care and appropriate support services post-discharge.Requirements:

Qualified Social Worker with experience in hospital discharge planning.
Registered with Social Work England (or eligible for registration).
Strong assessment and decision-making skills.
Excellent communication and interpersonal skills, with the ability to work collaboratively.
