Entegrity's Homecare Solutions is currently seeking experienced and compassionate LPN's to join our team. As an LPN, you will provide high-quality nursing care to our clients in their homes. This role requires strong clinical skills, excellent communication, and the ability to work independently in a home setting.
Responsibilities:
Administer medications and treatments as prescribed by the healthcare team
Monitor and assess client's condition and report any changes to the healthcare team
Assist clients with activities of daily living, including personal care, mobility support, and meal preparation
Educate clients and their families on healthcare management and disease prevention
Document client care and maintain accurate records
Collaborate with other healthcare professionals to develop and implement care plans
Provide emotional support and compassionate care to clients and their families
Requirements
Requirements:
Valid LPN license and registration in the state
Prior experience as an LPN in a home care or similar setting
Strong clinical skills and knowledge of nursing practices and procedures
Excellent communication and interpersonal skills
Ability to work independently and make sound judgments
Compassionate and caring nature
Reliable and responsible
Flexibility to work various shifts, including weekends and holidays
