Requirements
What You’ll Do
In our pivotal role of Patient Care Navigator at our primary facility in Jamaica, Queens, associated with the Child Center of New York, you’ll be instrumental in ensuring a quality patient experience. Your responsibilities include welcoming patients and visitors, identifying their needs, and guiding them appropriately. You’ll support answering phone calls, manage appointment schedules through our systems, confirm patients' personal and insurance details, handle payments and issuing of receipts, and facilitate communication with healthcare providers via email and fax. 
Additionally, your role involves supporting patients with screening procedures in the waiting area, fostering positive relationships with patients, their families, and the public—working in tandem with other team members to enhance patient experiences and facilitate impactful interactions within the practice team. 
Under the direction of the Physician or Registered Nurse (RN), a typical day may include—
Providing assistance to Spanish-speaking patients in scheduling appointments on the phone and in-person, understanding medical instructions, and accessing healthcare services
Offering Spanish/English translation and interpretation services to ensure clear communication between patients and healthcare providers
Educating patients about available healthcare services and how to access them
Developing an individualized patient navigation treatment plan
Empathetically helping patients in understanding their health insurance coverage and available financial assistance programs
Coordinating with medical staff to ensure patient care plans are understood and followed
Assisting in arranging transportation for patients to healthcare facilities
Following up with patients to ensure they understand their treatment plans and adhere to follow-up appointments
Advocating for patients’ needs and concerns  
Handling confidential patient information with discretion and in compliance with privacy laws (e.g., HIPAA)
Preparing and maintaining reports and documentation 
Supporting the onboarding of new employees

Knowledge
Understanding of the healthcare system, including medical terminology, healthcare services, and insurance processes
Familiarity with community health resources and services available to underserved populations
Knowledge of patient rights and medical ethics, especially regarding confidentiality and HIPAA regulations
Awareness of cultural sensitivities and social determinants of health in Spanish-speaking communities
Skills
Excellent bilingual Spanish/English communication and interpersonal skills
Strong critical thinking skills, able to identify underlying problems and develop effective solutions for our patients and team members
Proficiency in MS Office, Google, or similar  
Ability to work independently and effectively within a team 
Proficient in organizing, planning, and prioritizing work effectively
Ability to use computer systems and healthcare software efficiently
Skilled in problem-solving and making informed, ethical decisions
Competence in providing educational and informational sessions to individuals and groups
Abilities

Ability to establish and maintain effective working relationships with patients, families, and healthcare professionals

What You’ll Have
Bachelor's Degree and 2 plus years' experience in Patient Coordination care, in compliance with New York Health Home Standards
Ability to work onsite in Jamaica, Queens Tue - Fri, 9am - 5pm ET; remote on Mon
Required: Bilingual fluency in Spanish and English
Prior experience as a Patient Navigator, Healthcare Coordinator, Case Manager or in a similar role
