Duty nurses assist our patients with transitioning home from the hospital and

provide the continuous care needed to remain at home with their loved ones.

We match our nurses to their patients based on skill, age, health condition,

and acuity level. Our nurses make a positive and lasting impact on their

patients' lives by providing one-on-one care ranging from 8 to

16-hour shifts.

Responsibilitiesâ¯â¯â¯Â

• Implementation
  
  of physiciansâ prescribed plan of
  
  care Â
• Patient assessments
  
  and coordination of
  
  care Â
• Administration of
  
  prescribed medication, treatments, and therapies Â
• Compliant,
  
  timely documentation of care provided in system of
  
  record Â
• Health promotion,
  
  teaching, and training of family
  
  members Â
• Responsible for
  
  accepting and writing verbal orders from physicians as required and for
  
  obtaining co-signature of Case Manager on all verbal
  
  orders
• Follow up with physician, client,
  
  family members, staff, and Case Manager on client problems, concerns, and/or
  
  changes in client
  
  condition  Â
• Performs
  
  other appropriate duties as
  
  assigned

Qualificationsâ¯â¯Â

• Must
  
  have and maintain an active, unencumbered license (RN/LPN/LVN) in the state
  
  in which the clinician will practiceâ¯â¯
• Current
  
  CPR certification from American Red Cross, American Heart Association, or
  
  BLSâ¯â¯
• TB skin test (current within
  
  last 12 months)â¯â¯
• Six months prior
  
  hands-on nursing experience preferred but not
  
  requiredâ¯â¯
• Must have reliable
  
  transportationâ¯â¯
