Job Title: Persistent Offender Lead Practitioner

Salary: £26,000 DOE

Clearance Required: Enhanced DBS with Child and Adult Barring & NPPV Level 2

Location:

Newcastle (commuting to a Newcastle-based office)

Role Overview:

Our clients Service is funded by the Northumbria Violence Reduction Unit, aiming to create a safer environment where individuals, families, and communities can thrive without the fear of violence. Covering the Northumbria Police Force area, this service focuses on supporting vulnerable children, young people, and adults who are at risk of or have been involved in serious violent crime. We are seeking a Lead Practitioner to support individuals aged 18+ identified as persistent offenders in Newcastle city centre to reduce offending behaviour, ensuring positive outcomes through targeted interventions, wrap-around support, and referrals

Key Responsibilities:

· Manage caseload of service users within a multiagency hub.

· Establish professional relationships with the police.

· Increase visibility and accessibility of the service.

· Conduct person-centered assessments and develop support plans.

· Complete robust risk assessments and management plans.

· Facilitate signposting and referrals to support services.

· Maintain accurate case notes and database entries.

· Utilize person-centered practices to facilitate change.

· Implement flexibility in service delivery.

· Collaborate with partners to offer timely interventions.

· Adopt a Whole Family approach to provide wrap-around support.

· Develop exit strategies in partnership with agencies.

· Safeguard service users and others.

· Achieve key performance indicators for the service.

· Engage in continuing professional development.

· Ensure compliance with relevant legislation and policies.

· Uphold workplace values in all interactions.

Essential/Desirable Criteria:

· DBS on update service is essential

· NVQ Level 3 in Health & Social Care or equivalent

· GCSEs (or equivalent) in Maths and English

· Proficiency in Microsoft Outlook Office programs

· Strong oral and written communication skills

· Adaptability to meet service user needs

· Experience managing a caseload of service users

· Ability to work with vulnerable adults with multiple needs

· Experience conducting assessments and developing support plans

· Knowledge of safeguarding issues for children and vulnerable adults

· Understanding of risk assessment implementation and monitoring

· Experience in multi-agency working and maintaining confidentiality
